Prefer state health departments, facility and chain announcements, court filings, insurer notices and established local health reporting; ignore rumors and unverified posts. Put the facility, chain or system first in the title with the state and the date. Use high severity for an emergency department or maternity closure, a pharmacy closure with under 30 days of notice, or a network exit affecting 50,000 or more patients.
